New guide to identify patients who would benefit from a shared care plan
A new guide has been created for clinicians to identify patients who would benefit from an Acute Plan (AP) and / or Personalised Care Plan (PCP).
Who needs an AP or PCP? The short answer is...
- An AP is for anyone who is likely to present to urgent care services, including emergency departments, after hours clinics, ambulance services and primary care teams.
- A PCP is for anyone who is working on goals and actions to improve their health and wellness, especially when there is more than one clinical team involved. PCPs allow patients and their health care team to share patient-centred goals and activities.
The plans can be read by any health care provider with access to HealthOne anywhere in the South Island.
The longer answer, is contained in the newly created guide for clinicians, which provide guidance about which patients could be prioritised to have a plan created. There are also some tips on how a care team might search for and work up plans with patients, who are most vulnerable in a crisis and who need advocacy or information sharing to support safe and effective urgent care. A brief reminder
Acute Plans can be useful to share information/signal patient needs in a crisis. They are used by acute services across the health system to deliver care that is appropriate for the individual patient in an acute exacerbation.
