Nominations open for membership of new Primary Care Capability Service Level Alliance

Canterbury Clinical Network is seeking nominations for the membership of the new Canterbury Clinical Network Primary Care Capability Service Level Alliance. 

The Primary Care Capability Service Level Alliance will be established in March 2018 to give increased focus to the integration of the patients’ experience by bringing together a number of integration elements and enablers across the Canterbury health system.

The SLA will provide leadership to activities that support coordination and integration across primary care, with a primary focus on providing strategic system leadership to enhancing and supporting the capability and capacity of general practice, and ‘future’ general practice (as opposed to the traditional models) that enables the provision of care that delivers a better patient experience. It is recommended that the Patient Experience System Level Measure will be championed by this SLA.

Meetings will initially be held monthly on a Tuesday afternoon. The first meeting of the group is due to be held from 3-5pm, Tuesday 6 March.

The membership perspectives that are open for nomination are:

  • 1 x consumer perspective
  • Community provider
  • Allied Health
  • CDHB Planning Funding (GM or Team Leader)
  • Population Health
  • Pharmacy
  • 2 x primary care (GP and nurse - rural and urban)
  • 3 x PHOs (Clinical Leads/CEOs or their senior delegate)
  • Rural Health Integration
  • Pharmacy integration
  • System Level Measures
  • ALT member/Sponsor
  • Maori Perspective
  • Pacific Perspective
