Primary Care Liaison Team

The Primary Care Liaison Team is a multidisciplinary team working part time to contribute to a more integrated health system in Canterbury. The team includes pharmacy, nursing, physiotherapy and general practice liaisons who are passionate about health system transformation and have wide and influential networks.

We provide a voice for primary care in service planning and are consulted about service developments that affect Older People. For example the Primary Care Liaison Team is currently involved in the redesign of the Older Persons Health and Rehabilitation service and has an on going commitment to the development and review of HealthPathways relating to older people.

The team's role is to identify and work with gaps in service and facilitate and support change. We act to encourage and maintain effective relationships between various primary, secondary, aged residential care and community care health professionals and groups, improving outcomes for older persons wherever they might need health care.
