CCN has released the first quarterly report (July to September) for 2017/18.

Highlights include nearly 95 percent of children recognised as obese being referred to a family-based nutrition, activity and lifestyle programme; 37 percent of people aged over 75 year old referred to the ‘in-home’ Falls Prevention Programme; and 59 Primary Care Groups engaged in the Integrated Family Health Service (IHFS) to explore ways to work together and free up more patient focused care.

Other highlights and items of note include:

• A draft Hurunui Model of Care has been endorsed by the ALT and will progress to community and provider feedback phase during November/December 2017.
• A draft Oxford Model of Care is being further refined by the Oxford and Surrounding Areas Service Development Group.
• Work is proceeding on the Community Services Redesign, with three workstreams now meeting:
  • Referrals group
  • Visibility of InterRAI group
  • The Way We Work workgroup
• An additional $535K being provided to general practice for the care of their patients with complex needs since the implementation of Enhanced Capitation because of a reduction in HUHC registrations.
• The survey of general practice on the use of their Enhanced Capitation funds has provided some initial comment to inform the work of the Enhanced Capitation Transition Work Group.
• The Equally Well (EW) initiative is underway, to address health disparities of people with mental illness and/or addiction issues. The Canterbury Regional Equally Well Committee, including representation from the Mental Health Workstream (MHWS) and from across the whole system, oversees implementation of Equally Well. In Q1, 1055 Equally Well extended consults we offered to 974 clients.
• Summer Studentship Project under the Integrated Diabetes Service Development Group is set to start in Q2 to look at reasons behind people not participating in funded diabetes retinal screening programme.
• Sport Canterbury has been appointed as Lead Agency by ACC to co-ordinate the provision of circa 12,000 places at community strength and balance classes over the next three years
