Our first report for 2016/17 has shown progress in many areas.
Highlights include reduced ED presentations as a result of a fall, reduced High Use Health Card (HUHC) registrations following implementation of the Enhanced Capitation funding distribution model, plus ongoing usability improvements and increased rates of Acute Plans.
