About the Canterbury Clinical Network and Pegasus Health

The Canterbury Clinical Network (CCN) is a collective alliance of healthcare leaders, professionals and providers from across the Canterbury health system. We provide leadership to the transformation of the Canterbury health system in collaboration with system partners and on behalf of the people of Canterbury. 

The CCN programme team coordinates the activity of the alliance, providing day-to-day operational support to the various CCN groups.

Led by the Executive Director, the small team of employees is the hub around which the alliance operates. Functions of the Programme Team include programme coordination, project facilitation/management, resource management, administration, reporting and communications support.

The CCN is hosted by Pegasus Health, which is a collaborative association of General Practitioners, Practice Nurses and the wider practice teams, and seeks to improve the health and wellbeing of our community.

 

About the Role

Collaborative shared care planning coordinates input from a range of professionals across community, primary and secondary care into a single proactive plan, which is electronically available across the health system.

The Collaborative Care team has developed and launched the Acute Plan (2016) and Personalised Care Plan (2018) and are now focused on embedding the use of shared care plans across Canterbury’s health system. 

This role is to work as part of the Collaborative Care team to support implementation and use of shared care plans across Canterbury’s health system with a specific focus on their use in primary care.

There may also be a requirement to support other CCN activity, as agreed with the CCN Programme Manager.

This is 0.8 FTE role based in Christchurch, Canterbury. 

 

Skills and experience

The successful candidate will be able to build strong relationships and work alongside providers to assist them to integrate the use of Shared Care Plans in their everyday work. You will seek opportunities to innovate and enhance the usability of the plans and work collaboratively with other project teams supporting primary care.

To succeed in this role you will need:  

  • To have worked in a clinical capacity, ideally in a primary care or community health setting;
  • An ability to build and maintain strong relationships and be a great team player;
  • Strong facilitation skills;
  • Effective written and verbal communicate skills;
  • An understanding of project management/change management;
  • A track record of working collaboratively to lead new ways of working, ideally within a clinical setting; and
  • High levels of personal initiative, and a focus on results.
