The Canterbury Clinical Network is calling for nominations for a Primary Care perspective on the Pharmacy Service Level Alliance. It is anticipated that the new member will take up the role in February.

Organisations are asked to nominate a candidate who fulfils the criteria sought for the role in the nomination form.

Self nominations are accepted. All nominations to be made by 5pm, Friday 21 December 2018.
